Here's how it works:

1. Purchase a Return-a-Pet packet

   Fill out the enrollment form supplying the following information:

     A detailed description of your pet

     Your contact information

     Secondary contact information (as many as you choose to give us)

     Any other vital information that you feel we should know to bring your beloved pet home

2. Send Return-a-Pet the enrollment form

3. Place your study stainless steel ID Tag on your pet's collar.

If you change veterinarians or move, no problem, just contact Return-a-Pet and update your information...no need to order a new tag.
